Confirmation of Year 12 Subject Choices

This sheet shows the subjects you have chosen for your Year 12 studies.

  1. Please sign below to show you agree with these choicesA parental signature is not necessary if there are NO changes
  2. If, in the light of your results, you wish to request a change to one of your choices, please delete the subject you no longer wish to study and add another one.  Your 4 choices must be available in different columns.  IF YOU HAVE MADE CHANGES, BOTH YOU AND A PARENT MUST SIGN THE FORM.
  3. If a subject below is highlighted, this indicates there is a problem with your qualification for this subject.  Please choose another subject for which you have qualified.  If you wish to discuss this change please telephone the School Office (01494 524955) as soon as possible.
  4. Please return this sheet to the marked box in the Queen’s Hall or to the School Office by 9.00am on Friday 23 August at the latest.
  5. If you are taking an enrichment subject it will be on your timetable in September.

Please be aware that it may not be possible to facilitate the changes you have requested and you will need to discuss the situation on Wednesday 4 September.

Minimum Entry Requirements: 

8 full GCSEs at Grades 9-4 including English and Mathematics at Grade 5 or above and a total point score from the 8 GCSEs of a minimum 47 points.   If there is oversubscription in a subject, selection will be made according to the School’s Admissions Policy followed by the highest UMS score in the required GCSEs.
